Is there a LaTeX document class or style sheet for IU doctoral dissertations?

See IU Thesis Document Class, an unofficial LaTeX2e document class for dissertations, which says it follows the University Graduate School's Guide to the Preparation of Theses and Dissertations for formatting all front matter (title page, signature page, dedication, acknowledgments, abstract, tables of contents) and provides several page styles for the main matter.
